Oops title should be WHY is there so much illness in China!
Only been living in Shenzhen for 5 months and already on my third illness! Headache fever soar throat etc. More sickness than in the past 15 years of my life in just 5 months.

But it's not just me. People at work both Chinese and Western seem to be off sick quite a lot and there there are always people coughing or looking tried/worn out.

Several factors compounded. Just spitballing here but:
1) strains of bacteria and viruses to which your immune system has never been exposed
2) poorer personal hygiene habits among the population you're now surrounded by - people sneezing and coughing openly into confined spaces not washing hands with soap or meaningful frequency (and then handling the things you touch and the food you consume) etc.
3) the large dense population you now find yourself in makes transmission of contagious disease far easier
4) an immune system weakened perhaps by a change in habits habitat and possibly pollution levels

China is still a developing country and it takes time education and infrastructure to improve issues related to unsafe water sanitation and hygiene. It's also one of the most densely populated countries on Earth which compounds these issues. You should expect to fall ill more often under such conditions.

Because there's a billion people and a large percent of them are filthy. They aren't educated to cover their mouth or bury a cough/sneeze into a bended arm. On any public transport its a competition to see how far you let shit fly from your body and into the air and on others. Every surface has been covered in snot piss and shit and you are touching it all day everyday. Nowhere has soap to wash your hands and if there is a bottle it's had water added so many times it's now just water. Companies don't like it when employees call in sick. They'd rather have you come in and spread it to everyone. I could go one for a while but ill leave it there. Moral of the story is wash your hands more often drink more hot water and your healthy will day day up.

This sounds very anecdotal to me. I lived in China for three years and only got ill once and I didn't feel the people around me where ill that regularly.
That being said it may be related to over use of anti biotics which has damaged peoples ability to fight infections naturally and may help to spread bacterial infections. Also if you don't have a good diet or exercise regularly you may end up with a compromise immune system.
Things like taking shoes off and washing underwear separately are likely to stop the spread of infections despite what Chinese people may think.

Where and how did you grow up? This is probably one of the biggest factors in how sensitive you are to environmental factors and/or susceptible to illness.
You'll have to excuse me while I slip into Curmudgeon Mode for a bit...
If you're young and from the US/UK/EU chances are you grew up in a bit of a protective bubble. Parenting over the past few decades has become more and more adverse to letting kids "go out and eat dirt". There's less exposure to natural bacteria and therefore lower natural tolerance to them (white cells need exposure to build antibodies).
Additionally young people now tend to be less tolerant of illness. As a manager I see young people calling in "sick" for things people my age wouldn't even acknowledge. This is also what I see in Chinese people. "I have a slight cough. I need to go to the hospital for an IV." Seriously? Slam a shot of 44D and man up.
[/Curmudgeon Mode]

If you're young and from the US/UK/EU chances are you grew up in a bit of a protective bubble.
I'd say that's doubly true for middle class Chinese kids in the One Child Policy age. They grew up with insane homework loads and tutoring schedules and any free time they had they were generally led around highly domesticated spaces (city parks theme parks malls etc) by a chaperone team two to six deep (two parents and two sets of grandparents). When did these kids ever get to run around in the woods and eat dirt? We're talking about a demographic group with really high rates of poor eyesight almost certainly lixed to a lack of exposure to natural light (the homework thing again).
I'm right with you on the IV thing.

I guess that's also part of the problem. People just going in to work sick just spreads it around more making the cycle of sickness worse.

Curious how much evidence is there to back this theory up? I definitely agree I think exposing kids to germs is a good thing and builds up immunity.
Regarding Chinese hypochondriacs I agree with that as well. My wife went back for a month during Chinese New Year. Said her sister took our infant nephew to the hospital at least five times for various minor coughs.

“Curious how much evidence is there to back this theory up? I definitely agree I think exposing kids to germs is a good thing and builds up immunity.”
There's quite a bit. Look up "Hygiene Hypothesis" for one example.
Here's on study listed at NIH.gov:
The mammalian host immune system is broadly stimulated with the first exposure to microorganisms during neonatal life (1). The inner surfaces of the gastrointestinal tract and lungs are particularly affected because they are predominant sites of microbial contact (2). Epidemiologic observations further suggest that the immune effects of early-life microbial exposure are durable and persist into later life because they can be associated with prevention of diseases such as inflammatory bowel disease (IBD) and asthma (3–5).

Another thing is the way the medical system works here. Even for simple things people go to the massive hospitals and get an IV drip which necessitates time off. They get pumped with antibiotics and god knows what else just for the common cold where people in the West would just pop a few cold pills and rest a bit more.

I once saw a man washing his hands up to the elbows in a puddle on the side of the road then jump on the bus grabbing every handles on his path.
After that I start using hydroalcoholic gel to the point it was hurting my skin.
I think diseases spreading is mostly due to people general lack of knowledge about good practices on hygiene. In my office they're all wearing a mask when they're sick to avoid giving their cold to others.

I lived in the north of China before and it was especially true that everyone -foreigners and locals- would be getting sick often. I think this is a mix of many things: pollution overuse of antibiotics lack of education in regards to things like food safety/hygiene etc. I'd also seen an article somewhere that purported that bacteria live and dead is able to travel with the pollution and transmit easier.
I'd also argue that the use of cheap/low quality materials can contribute to the problem. IE. The plumbing in apartments or the sealing of windows. I'm not surprised when I see mold in an apartment or if it smells musty.
Then again I've recently moved near Shanghai and I wasn't sick at all this winter. The pollution is a huge factor IMO.
