Seven of the top 10 most profitable hospitals in the United States are nonprofit facilities that each netted more than $150 million from caring for patients in 2013, according to a study published Monday.

Topping the list is Gunderson Lutheran Medical Center in La Crosse, Wisc., which earned $302.5 million in profit from patient care services that year, according to researchers from the Johns Hopkins Bloomberg School of Public Health and Washington and Lee University.

Other nonprofits in the top 10 include the Stanford Hospital in Palo Alto, Calif., which took in nearly $225 million, and the University of Pennsylvania's hospital in Philadelphia, which earned $184.5 million.

"Most hospitals lose money, but there are a few very profitable ones and we need to pay attention to why they are making so much and how it affects consumers‘, said lead author Gerard Anderson, a Hopkins health policy professor.

In study in Health Affairs, Anderson and co-author Ge Bai, an assistant accounting professor at Washington and Lee, only analyzed net income for patient care services for fiscal 2013, the most recent year for which data were available. They didn't include profits that hospitals earn from other activities, such as donations, investments, parking fees, rental space and sales from gift shops, which often are used to subsidize patient care.

"All hospitals should make a little profit," Bai said, "but some hospitals are obtaining outrageous profits."

In a statement, Gundersen said the report does not reflect its costs as an integrated health system in a largely rural, multistate region with a large Medicare population. It said the data used by the authors excluded some administrative and other costs that would have changed the medical center's ranking.

The study's main purpose was to determine the characteristics of the nation's most profitable hospitals. They found that facilities that were part of a system were more profitable because they were able to dominate their local market, which gives them greater clout in negotiating higher prices from private insurers. It also means consumers end up paying more if their health-care provider is out of network, Anderson said.

The hospitals with the highest price markups earned the largest profits. Anderson thinks they should lower their prices or plow more back into the communities.

"Mostly, the hospitals are able to charge more because they can, and they do," he said. "There's no need for nonprofit hospitals to earn substantial profits."

Hospitals are among the largest property owners and employers in many communities. Those designated as nonprofits receive state and federal tax breaks for providing “charity care and community benefit.”

The value of tax breaks for nonprofit hospitals has doubled over the course of the past decade, research shows, adding fuel to a longstanding debate on whether the facilities give back enough to the public. The report suggests that policymakers consider whether nonprofit hospitals should be required to invest profits in additional services or lower their prices to justify their tax-exempt status.

Non profit hospitals make money, as is said in the article, outrageous money. These things needs to be looked into for sure. They could be used to bring down health care costs.

What are the locations of these 10 hospitals? Some are given in the article and others are obvious, but there's a lot of Methodist hospitals out there, especially in the South. Readers should NOT have to guess which one made a mint in 2013, especially since the one listed is for-profit (many if not most Methodist hospitals remained not-for-profit even after they were spun off from the church -- Methodists, like Baptists, mostly spun off their hospitals in the mid-1960's due to Medicare & Medicaid conflicting with their views on separation of church & state).

Why isn't the headline "Most Hospitals Lose Money"? I think that's the biggest eye opener in this story.

Until the 1980's when hospitals hired MBAs no one thought it was a good idea to ream the federal government for maximum billing.

But all at once the MBAs increased hospital billing, promoting themselves to CEO - passing out hundred thousand dollar raises to the hospital administration ... and staggering the federal government.

The hospital I knew went from $5 million in the red to $50 million in the black in one year.

How did the federal government respond? Stupidly...put another set of MBAs in charge with another useless layer of managed care that locked in those high charges... and took their ever-increasing slice.

You might know what it is like to have one person's work support twenty. But these hundred million dollar profits at select hospitals don't touch the billion dollars being put away at little managed care Health Insurance Companies.

They are putting away trillions this year.

Stanford's in the middle of building a much larger hospital--the community has far outgrown the old one that has been added onto wing by wing since the '50's, and the state passed a law a few years ago requiring that hospitals be able to stay standing in the most extreme earthquake scenario with a deadline for compliance that is coming right up.

So when they said capital improvements, I believe it--I've seen the huge amount of construction going on.

And it's a huge relief, because I was a seriously ill patient on a waiting list for a hospital bed there in the last ten years. They told me I could be admitted but would be put out in the hallway till someone else made space and there would be no telling for how long. The answer, it turned out, was nine days. They were simply jammed full.

When I finally did get in the care was absolutely top-notch. As an example, I randomly ran into one of my Stanford doctors several years later. He was thrilled to recognize me, to see me out and about and healthy now, and he recited from memory every med I'd been on, both experimental and not--I as a patient had mattered to him enough that he had remembered.

The "nonprofit" designation merely means that the company cannot distribute profits as dividends to shareholders. Nothing more or less than that. It means an outsider cannot invest in the company as a shareholder and be entitled a piece of that profit, it has nothing to do with whether the company itself can make a profit. It can still keep as much profit as it thinks it can get away with, overpay the CEO, etc. etc. just like any for profit corporation.

Every business, nonprofits include, has to make a profit over the long run to survive as a going concern. And no, nonprofits aren't automatically tax-exempt either.

Hospital profits would be a whole lot higher if they didn't grossly overpay their executives. You can make a lot more money as a non-profit hospital executive than as a public corporation executive of a similar sized non-hospital business. The execs get paid when the hospital loses money too.

I worked for one of the hospitals on that list for over a decade. Although the hospital is not for profit it partners with a for profit clinic owned by the physicians. They give outstanding care and I loved that job. It doesn't matter how profitable the hospital is the important challenge is the quality of the care they provide to patients. In that they did an outstanding job in the level of care they provided.

This is about tax exempt hospital and their executives ''enriching themselves'' off of the system. Great care can be given without health care institutions making hundreds of millions of dollars off the backs of their patients. This is the point of the article.

I'm not commenting to defend hospitals, but I'm very skeptical about such a shallow look at hospital profit and loss statements. Ask any CPA to analyze the P +L statement and balance sheet of a big business and get comfortable, because conclusions require a lot of explanation. In any complex business accounting rules can cause an organization's statement to be way up or down due to the rules. In some cases these hospitals may have had one time situations that inflate their net, or they may be saving money for a reserve in a good year to cover the bad years, or they may be saving up money to purchase new buildings or equipment in the next budget year.

In 2012, average cash compensation for CEOs of non-profit hospitals was $2.2 million. Average CEO salary for a mid- to large-size public charity was around $125,000. The hospitals state the job is so big and complicated, they have to pay millions to retain their CEOs. The article notes that the president of the U.S. makes $400,000 -- certainly an even tougher job.

The ability doctors and nurse have to relieve suffering and extend meaningful life is being dwarfed by a culture of financial opportunism. The people on the ground who provide day-to-day care are withering in this storm of undeserved discontent. Stand in the shoes of your local doctor and nurse and you will see the true calling and meaning in the healing professions. And if you yourself are sick and in need, you will understand the irony of having great progress but limited understanding of what good medicine can do for us all.

One would think that a 'non-profit' when faced with a surplus would lower the cost of the patient services but that is rarely done, instead the 'non-profits' go on a spending binge buying and building things that aren't even core services. The local 'non-profit' is currently in a fight with locals because it wants to take a huge parcel of the little remaining farmland that was donated to them and build homes as if they were developers. The proximity of the land could provide healthy, fresh organic food for the hospital but that seems really dull compared to building new oversized homes, how this will help keep people healthy is yet to figured. And someone please explain how the local 'non-profit' health insurer and 'non-profit' health networks all have executives boxes at every new arena . . . .
