A Chinese co-pilot is recovering in hospital after apparently being sucked out a cockpit window and had half of his body dangling out of the plane at 32,000ft high.

Xu Ruichen, 27, was saved by his seat belt after the windshield in front of him suddenly broke and fell off the Sichuan Airlines plane carrying 119 passengers, said the captain of the flight to Chinese media.

This happened in Britain in the 80s or 90s - British Airways I believe. They found out that they had used the wrong size bolts on the windshield. The pilot survived and is still flying (for a different airline) IIRC.

OK, all these window problems may be related. Too many in such a short period. It's not the manufacture because one was Boeing and the other is Airbus. Is it the maintenance?

I work in airport operations, and this incident as well as the incident with the lady being pulled out of the window are the only ones I've ever heard of. There are a lot of mechanical issues I've dealt with. People dying on the plane. Bird strikes are extremely common, but I have never ever heard of window issues (except for birds strikes) until now. Even in the case of bird strikes cracking the windshield, they have always remained intact. I agree that something weird is going on.

Can only be a maintenance issue for the whole windscreen to pop out - must have used the wrong sized bolts as happened to another aircraft in the past

In this case, the windshield had never been replaced. So it wasn't the wrong bolts, as had happened in a previous such occurrence. The aircraft had seen 12,920 take offs and landings prior to this flight. An ECAM message about windshield overheating was received. Perhaps the windshield heater had stuck on, causing thermal stress.

Unlikely. Pilots tend to only use the 4 point harness during takeoff or landing, or turbulence. At other times they use a lap belt the same as the passengers.
